The Office of Student Conduct has developed numerous resources to aid University faculty, staff, and administrators in filing student conduct complaints against students for violating a University policy. These resources are available in the Student Conduct Office in Wood Support Services, Room 247 or can be downloaded below.

Academic Misconduct Report Form: A faculty member who has determined that academic misconduct has occurred in a course at Eastern is required to document the incident, meet with the responsible student(s), and inform the student(s) of the sanctions imposed. If a sanction imposed is a failing grade for the course, the faculty member is required to submit a copy of an Academic Misconduct Report to the Director of Student Conduct and the Registrar's Office within three business days of meeting with the student(s). Faculty members who impose a sanction less than a failing grade for the course are encouraged to submit an Academic Misconduct Report form and supporting material to the Director of Student Conduct.
Complaint/Referral Form: Use the complaint/referral form to report alleged non-academic violations of the Student Code of Conduct. Please note that fields marked with an asterisk are required. If you have questions, please contact the Office of Student Conduct at (860) 465-0063.
